Why Are Hospital Floors a Slip Risk?
Hospital floors carry a heightened slip risk because they combine constant contamination, frequent cleaning and disinfecting, varied footwear, and a population that is often less steady on their feet than average. Patients, residents and visitors are more likely to have reduced mobility or balance issues, which increases both the chance of a slip and the severity of any resulting injury. A quiet incident log doesn’t mean this risk isn’t present, it often just means it isn’t being reported.
The short why
Every one of the six factors that drive slip risk tends to run harder in a hospital than in a typical commercial building. Spills and bodily fluids are routine rather than occasional. Cleaning happens far more often, which increases the chance that a product is quietly affecting grip without anyone noticing. And the people walking those floors are, on average, more vulnerable to a fall than the general public. Together, these factors make hospital floors genuinely higher risk, even where nothing has gone wrong yet.
What actually drives the risk
We use a framework called CHIMES to explain what makes any floor slippery, and hospitals score poorly on several factors at once.
- Contamination: spills, bodily fluids and cleaning residue are a constant presence, not an occasional event.
- Individual: patients and residents are more likely to have reduced mobility, balance issues or medication side effects that affect both stability and reaction time.
- Maintenance: the sheer frequency of cleaning and disinfecting increases the chance that a product is leaving a residue that reduces grip, even while meeting infection control standards.
- Environment: transitions between wards, poorly lit corridors, and matting that doesn’t extend far enough near entrances all add risk on top of the floor itself.
- Surface: many hospital floors, particularly in older buildings, were specified for durability or infection control rather than tested slip resistance.
No single factor explains hospital slip risk on its own. It’s the combination, happening constantly, that makes the environment genuinely harder to manage than most.
Why no incidents does not mean no problem
This is worth challenging directly, because it’s where many estates teams get a false sense of security. Near misses in hospitals are especially likely to go unreported. A patient who catches themselves on a handrail, a visitor who slips but isn’t hurt, a nurse who steadies someone before they fall, none of these typically make it onto an incident log, even though each one is a genuine warning sign.
A quiet log can mean the floors are genuinely safe. It can just as easily mean the warning signs simply aren’t being captured, particularly in a busy clinical environment where staff are focused on patient care rather than incident reporting. The only way to know for certain is to test the floor’s actual slip resistance directly, using a pendulum test that produces a Pendulum Test Value, or PTV.
Why this matters more than in most other environments
The consequences of a fall in a hospital or care home tend to be more severe than in a typical commercial setting, given the population involved. Organisations that move from assuming their floors are safe to actually measuring, improving and proving slip resistance typically see incident reductions of 57 percent or more, based on data from Slip Safety Services across thousands of sites. In a healthcare setting, where a fall can mean a serious injury for an already vulnerable person, that difference carries real weight.
What to do about it
Start with the highest risk areas, bathrooms, ward entrances, and corridors near sluice rooms, since these combine the most contamination with the most vulnerable footfall. Get these tested with a proper pendulum test rather than relying on how clean or dry they look. Encourage staff to report near misses as useful information rather than something to downplay, since this is often the earliest signal available before a full test can be arranged. Build a retesting schedule so any change in performance is caught early, rather than discovered after an incident.
